NCHP is a hospital organization consisting of four area hospitals in Northern New York State. NCHP has developed this web site in order for the five community hospitals to better communicate and provide useful information to the public served by the organization. The NCHP web site offers both internet and intranet services allowing physicians and other health professionals to utilize web technology to determine the in house resources of each facility, communicate between facilities and quickly identify the ability to provide the services within the network or whether outsourcing is required.

Our Mission:

"Develop an integrated delivery system, which offers a full continuum of care, creates a partnership among providers, employers and the community health improvement, provides a focal point for medical management activities and managed care contract negotiations, maintains local medical care delivery, and, maximizes the number of individuals served."

Our Strategy and Purpose:

The stated goals of NCHP are to improve employee health at lower cost, to initiate disease management efforts, to improve physician partnership through a coordinated delivery system, to reduce the outmigration of patients, and to establish a NCHP data warehouse.

The NCHP Network:

The North Country Healthcare Provider network area of service consists of St. Lawrence County and Northern Franklin County, representing a population of about 160,000 persons. The area is highly rural in nature characterized by communities with small populations (below 2,000), low median incomes and education, and significant problems in accessing services due to distance and weather.
